Amdal In-Home Care Enhanced Care Services

In addition to our basic care, your Care Coordinator is available to provide a wide array of additional services on an as-needed basis.   Some families need us to become more involved in their loved-one’s affairs, and we can step in as requested.  Examples of this include: 

In-Depth Nutritional Assessment and Recommendations Review of nutritional requirements, existing diet, medical conditions and recommendations for dietary improvement.
Nutrition Management Program Complete management of client nutritional and fluid intake, from assessment of needs, to planning optimal diet, grocery shopping and meal preparation.  This program includes preparing nutritious, appropriate meals in the home.
In-Depth Environmental and Home Safety Assessment and Recommendations A complete review of the home environment, inside and out, to identify hazards and safety concerns, and to recommend changes or improvements for optimizing the home environment for safer senior living.
Environmental Improvements Coordinating improvements to the physical environment in accordance with environmental assessment, or other modifications required by client’s family
Mini-Mental Exam This exam serves as a "snap-shot" of basic mental functioning and may identify changes in cognitive ability if performed at regular intervals.
Medical Record Review, Consolidation, Creation of "Vial of Life" Critical Info packet for EMT's We can assist you in identifying critical records and information, and in assembling it all in one easy to access place.  This also includes the creation of an "important emergency information” document to be placed in a ready location for EMTs or other healthcare providers if needed.
Medication Management Solutions If needed, Care Coordinators can order meds, consolidate ordering and medication inventory, purchase med boxes, pick up and deliver medications.
Petty Cash Management You can have Amdal In-Home Care maintain small amounts of petty cash for the purchase of food, personal items and other household expenses, manage and transmit receipts to you, and provide a complete accounting of all expenditures.
Health Care Professional Interface We can act as a patient advocate at physician’s appointments, document key interactions with medical providers and communicate the interactions to family as needed.
Insurance Interface (non-billing related) While we obtain LTC authorizations as a basic part of our service, there are times when family requests we interact with other insurance professionals, such as case managers, to facilitate the verification needs required by the insurance.
Durable Medical Equipment Facilitation We can interface with client’s health care professionals to obtain physician's orders for necessary durable medical equipment, and also make sure it is ordered and installed.
Ordering and Maintaining Personal Supplies If there are other personal care items the family wants us to inventory and order, we can do that.  Examples could include adult diapers, gait belts, gloves, etc.
Household Services Management We can manage other services related to keeping the household environment safe and smoothly functioning, such as yard work, bathroom modifications, plumbers, etc.
Assisted Living/Skilled Nursing Placement We can evaluate and assist the family in finding permanent placement most appropriate to the client’s current needs.
Referrals to Home Health, Hospice or other Health Care Entity As needed, we can interface with the physician to obtain orders for home health, hospice, physical therapy, skilled nursing, etc.  As we are in the home, we often are the first to identify a need and can facilitate the commencement of these services.
Managing Transfers home from Medical Facility Let us handle the complete transition home from a medical facility, adapting the home environment to best fit the client’s current need.  We can transport and stay with the client as he or she acclimates to being back home.
